Address Changes

  • Home or Local Addresses:
    AUHS Student Services Office requires students to submit a local address. Current students can change their home or local address by sending a signed letter to the Office of Student Services. The letter must include name, student ID number, signature, and the new address, specifying whether it is the home or local address.
  • Parent Address:
    Students who are "dependent" cannot change their parent's address; however, parents can submit a change of address to 3501 Atlantic Ave, Long Beach, CA 90807.
   
Personal Data Updates

If the Students need necessary corrections about their personal data, please use the form provided to update your information and mail it to AUHS Student Services Office, 4300 Long Beach Blvd., Long Beach CA, 90807.

 
   
 


School Identification Numbers

All AUHS students are assigned a student number. This School I.D. number will be used for all school records including registration, class adjustments, and access to school equipment and library.

School ID is issued for new students on the 5th day after their start date. Photo identification must be presented to receive a new card. Valid identification includes a driver's license, state identification card or passport. The AUHS ID card is valid for on-campus identification only.

Replacement I.D. Cards

Replacement ID be obtained Monday through Friday, 10:00 a.m. - 5:00 p.m. from: Student Services Office, 3501 Atlantic Ave, Long Beach, CA 90807.

Photo identification must be presented to receive a new card. Valid identification includes a driver's license, state identification card or passport. The replacement fee AUHS ID cards is $10.

Copier Codes

Each student is given a copier code on their 5th day in class included in the student ID. Students can have additional codes and these can be requested at the Student Services Office for $5.00/ 100 copies and $10.00/ 300 copies.

Withdrawals and Leaves of Absence

A student who is registered for a program and who decides not to attend must file an Official Withdrawal/Leave of Absence form. The Withdrawal orLeave of Absence is effective on the date the form is received. Withdrawals are initiated when the student wishes to permanently leave the University. Leaves of Absence, however, are taken when the student wishes to leave the University, but intends to return in a future class or module. All students who wish to withdraw or take a leave of absence must file an Official Withdrawal/Leave of Absence Form. Students who wish to file Withdrawal or LOA can download the form below but must schedule an appointment to visit the Student Services office or mail the application form with proper signature.
